Requesting Materials
Before requesting slides or blocks to be pulled, please do the following:Contact UW Medicine Pathology clinician to arrange review of the slides/blocks before they are sent out. Then complete and submit the forms listed below and send them to the facility you are requesting the materials from (location contact information is listed on the forms):

Please note that it is UW Medicine Pathology policy not to release surgical tissue blocks and original slides for research purposes, unless deemed appropriate by a pathologist. It is the responsibility of the researcher to make arrangements for a University of Washington Medical Center/Harborview Medical Center pathologist to review and approve requested materials prior to submitting a request. Re-cuts can be made in our histology laboratory.
